PEIA Worksite Health Screen

(Marshall Recreation Center)PEIA will be offering the Annual Worksite Wellness Screen on Wednesday Oct 5th from 7 am till Noon in the Marshall Recreation Center.  Please call 696-4103 to make an appointment.  Tests available for PEIA members are:  Blood Pressure, Waist Circumference, Total Cholesterol, HDL/LDL, Glucose and Triglycerides.  PSA is available with physician order.  For best results, 8-12 hours of fasting is recommended.
 
*If insurance is not PEIA primary, The Health Plan or Humana, there will be a $20.50 charge for the lipid panel and glucose test.
 
Other tests are available for a small fee:  Hemoglobin A1C -$14.00; TSH- $20.00; Comprehensive Metabolic Panel $15.50; Complete Blood Count $10.00.
 
To take part in the Lifestyle Change Programs incentive, individuals must participate in a health screen.
